This session will discuss how the HIT agenda needs to be conceptualized for health care reform with an eye toward consumer needs for information as well information technology. Following a framing of the public policy issues on information sharing, current and past Capitol Hill staffers will share their perspectives on strategies to leverage HIT and patient-centered care in health care delivery redesign as Congress prepares for the 2009 health care reform debate. They will discuss legislation focused on improving quality, HIT adoption, patient-centered care, and access for underserved populations.

He will approach these questions from two perspectives: His role as Chair of the Institute of Medicine Roundtable on Health Literacy; and as the person in charge of ensuring high-quality care in a large health care delivery system.

Track One: The Role of Ix in Enabling the Patient-Centered Medical Home
Chair: Rushika Fernandopulle, MD, MPP, Co-Founder, Renaissance Health

A patient-centered medical home (PCMH) has the potential to encourage a much more comprehensive approach to primary care delivery. If implemented successfully, the PCMH model can ensure access to high-quality, coordinated, and integrated care.

Successful implementation requires focus on effective communication with, and engagement of, the patient. Specific Ix and patient-centered HIT initiatives have been designed specifically to create more continuity and integration of care, and to engage consumers as active partners in care management. Faculty from a wide range of practice settings will share in-depth examples from their PCMH-Ix implementation experiences.

The objectives of the PCMH Track will be to:

Define PCMH and explain the role that proactive engagement of patients in primary care management through tailored information has in PCMH success.

Provide in-depth examples of how PCMH has been implemented through patient-centered HIT strategies in different types of practices settings.

Discuss the role of payment structures and measurement in advancing PCMH evolution that ensures future primary care has a true patient-centered orientation.

Dr. Barr has spearheaded patient-centered medical home innovation for ACP-the largest primary care medical society in the untry. He will share his vision for what PCMH should look like and what early PCMH experiments have taught us about its implementation in different kinds of primary care settings. Dr. Eytan has spearheaded patient-centered health information technology for Group Health Cooperative, and more recently, as part of the Center for Information Therapy's efforts across the United States. He will share his findings (and optimism for) a shift of focus toward patients in HIT implementations to support people receiving care where they live, work, and play, consistent with PCMH principles.

In this session Kerry Meyer will share her insights and experience from working with patients in one of the few real, working Medical Home pilot projects, serving complex-employees and dependents of the Boeing Company in the Seattle area. She and one of her patients will discuss the role Ix plays in their practice, what is working to improve outcomes and reduce total medical spending, and what barriers they have had to overcome.

Friday, June 13, 2008 - Track Two

Track Two: Ix & Health Disparities

Chair: Warren Jones, MD, FAAFP, Executive Director, Mississippi Institute for the Improvement of Geographical Minority Health

Health literacy, numeracy, the "digital divide," and other health disparities create special challenges in designing Ix and patient-centered HIT initiatives. Conversely, Ix and patient-centered HIT create unique opportunities for addressing health disparities. This track will address both the barriers and the promising new interventions that have been developed to narrow the disparities and improve health for safety-net populations

Innovative patient-centered HIT applications employ audio, video, graphics, plain language, and mobile phones. They leverage access to digital media through community public resources. Faculty will share research and experiences working directly with safety-net populations to implement solutions that meet the needs of their particular constituencies.

The U.S. Department of Health & Human Services and stakeholders around the country are building an ambitious agenda for the country to use health communication and Health Information Technologies (HIT) to overcome health disparities and improve health and health care delivery for everyone. This session will highlight the process by which HHS is incorporating health communication and HIT into Healthy People 2020. HHS will also report on the implementation of related "expanded care model" strategies at community health centers across the country. For a private sector perspective, WellPoint will share its experiences in health improvement outreach to a diverse range of Latino populations.
