Karen Kraemer, RN, CMC
Karen Kraemer serves as the senior director of Case Management for HealthPartners. She is responsible for developing and implementing Inpatient Care Coordination and Outpatient Case Management programs within HealthPartners.
Ms. Kraemer has been with HealthPartners for more than 22 years, holding leadership positions first within the HealthPartners Medical Group and later with the HealthPartners corporate organization. Prior to joining HealthPartners, Ms. Kraemer served with Metropolitan Medical Center of Minneapolis in orthopedics.
Ms. Kraemer is an R.N. graduate of Lutheran Deaconess Hospital School of Nursing in Minneapolis. She obtained a Bachelor of Arts degree in business from Metropolitan State University, St. Paul. Ms. Kraemer is Case Management Certified by the American Institute of Outcomes-Case Management and is also a member of the Case Management Society of America
